The Case Manager in the Family Support Worker role assists families who are caring for a child with a developmental disability or dual diagnosis to navigate a variety of services to ensure adequate support and future planning are addressed.

The Case Manager is responsible for providing the client and family education and assistance related to community processes and services, working closely with involved community partners and advocating for the client's needs.

All referrals for this program are through Contact Brant. Specific criteria for this program include a child with a primary diagnosis of a developmental disability and a resident of Brant County. This is a voluntary service.

Services are provided to clients on an ongoing basis until the client no longer requires service or declines service.
