Community Integration Services

(FOR ADULTS WITH A DEVELOPMENTAL CHALLENGE)

The purpose of the Community Integration Services is to assist adults who have a developmental challenge to live as normal and independent lives as possible within a community setting through ensuring they get all the appropriate and necessary services and that their rights are not compromised.

Community Integration Services, also known as Adult Protective Services, provides support for the clients in the community,  and assumes greater significance as the Ministry's effort at de-institutionalization, normalization, and inclusion are implemented.

All referrals for this program must come thorough Contact Brant.  Specific criteria for this program include:  18 years of age or older, a primary diagnosis of a developmental disability, and a resident of Brant County.  This service is entirely voluntary.

The Community Integration Workers assess the individual, his/her living situation, and their level of skills and needs.  They act as advocates or liaise for the client in matters relating to access to housing, education, vocational, financial, medical and legal needs and to ensure that their rights are not compromised.  They provide counselling for minor difficulties the client may have adjusting to community living.  The workers assist the client in developing basic living skills such as budgeting, nutrition, transit travel, and learning their rights and responsibilities on an individual basis, or through available groups.

Services are provided to regular clients on an ongoing basis and the client could maintain access to the services for as long as she/he requires them, or as long as she/he wants to receive services.

The Community Integration Workers may also work with the Family Support Worker to coordinate services offered to individuals as they transition towards independent living.
